Gynecological Services explained
Annual visit includes comprehensive review of your histories, full physical exam including breast/chest exam with pelvic if indicated/requested, screening services, relevant lab work, including blood tests, pap smear, STI/STD screening and testing and needed referrals for Ultrasound/Mammography and/or Thermography if indicated/desired
Problem visits include reviewing the “problem” and establishing a plan of care
e.g. abnormal vaginal discharge, relevant swabs/testing, results review, treatment options and recommendations, prescriptions as needed
STD/STI Screening/Testing includes history review, risk assessment, lab work (blood/swabs/urine), results review, treatment options and recommendations, prescriptions as needed
Preconception/Fertility Counseling visit includes history review, health goals and recommendations, discussion of relevant supplement support, herbal remedies, medication management and optimal nutrition/lifestyle recommendations
Comprehensive Contraception Counseling visit includes review of options/methods, individualized recommendations and prescription services as indicated

At East End Midwifery services we do not participate in-network with any insurance providers. We accept payment according to our schedule of fees for services and will provide you with a bill with diagnosis codes to submit to your insurance carrier for reimbursement. We do not guarantee that this will lead to reimbursement, that will be determined by your insurance carrier and what your out of network benefit package includes.
Comprehensive Prenatal, Birthing Services and Postpartum Care
Prenatal care, birthing services and postpartum care with East End Midwifery Services is a comprehensive model that allows for intimate care from beginning to end. We look forward to developing a relationship with you and your support team to build trust leading up to your birthing time. Embarking on a journey to home birth is one that involves a holistic approach to prenatal care; not only ticking the boxes with regards to blood work, sonograms and visits but including the emotional and psycho-social support it often requires to be free to birth and subsequently parent in an empowered way.
Retainer Fee: $2000 out of pocket

Your out-of-pocket retainer fee secures your place in the practice and covers expenses related directly to your individualized care.
If you come into care with EEMS (regardless of your gestational age at the time) you can expect a minimum out-of-pocket fee of $2000, this is accepted in separate payments of $1000 dollars at initiation of care and again in your third trimester, prior to 36 weeks gestational age.
Because we are out-of-network with all insurance carriers we operate utilizing balanced billing. With the help of a 3rd party billing service, we submit to your insurance for a global care fee. Depending upon the rate at which your insurance carrier reimburses, you are responsible for ensuring that EEMS receives the agreed upon fee for services, which can be quoted during a Meet and Greet. If your insurance reimburses at or above previously agreed fees and you have paid the retainer in full, your financial obligations are complete. If your insurance carrier reimburses below the agreed sum, you are responsible to make up the difference.
Typically your insurance company forwards you the reimbursement check and you then sign it over to EEMS or write a new check directly to us. Occasionally insurance companies reimburse at a higher rate than is typical for home birth. All reimbursement from your insurance provider is to be paid directly to EEMS regardless of the amount.
